Obtaining the Application
For information about the application process and to apply, click here.. It is advisable to do this a month before your birthday, since subsequent annual certification renewals are automatically sent out by the State before your birthday. Note: ONLY Registered Dietitians are EXEMPT from proving that they have the required training and experience in the field of dietetics/nutrition and from taking a State-administered certification exam.

Completing the Application

- Request a letter from CDR (800/877-1600 Ext. 5500) to be sent directly to the State to provide official verification of registration.
- Complete the 2 page application.
- Obtain a passport photo to affix to the application.
- Have the completed application notarized.
- Obtain a money order or bank check for $190.00 payable to the "Treasurer, State of Connecticut." (The annual renewal fee is $100.00 and due on your birthday.)
